Job Summary and Qualifications

Role Summary: The advanced clinical pharmacist practices the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. In addition, the advanced clinical pharmacist is expected to lead pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences and contributing to publications, and providing regular, formal education to pharmacy and interdisciplinary teams. The advanced clinical pharmacist is responsible for all behavioral health depts in the hospital. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the clinical pharmacist.

What qualifications you will need:

• Required Education: B.S or Pharm D. degree in Pharmacy.
• Required License: Maintains current pharmacist licensure, Pharmacist Board Certification, and Certification in specialties for the position as determined within 1yr of employment
• Required Experience: Graduate of an accredited PGY-1 residency program
• Preferred Experience: Graduate of an accredited PGY-2 residency program, 5 years' inpatient hospital experience, or 3 years' HCA inpatient hospital experience.

Live Oak Mental Health and Wellness Center, is a part of Trident Health. We give inpatient stabilization treatment to patients who seek treatment for acute psychiatric symptoms which cannot be addressed on an outpatient basis. Common disorders of patients treated in this program include but are not limited to: major depressive disorder, schizoaffective disorder, schizophrenia, bipolar disorder, adjustment disorders, post-traumatic stress disorder, impulse control disorder and a variety of co-morbid personality disorders. Our patient population is both adult and adolescent. We offer 24 hour availability of a full range of services. We have the capability to institute rapid lifesaving interventions. Our physician availability is 24 hours a day to institute needed modification to the treatment plan. Psychiatrist involved in the direction and management of the treatment program. Round the clock skilled nursing care implementing the treatment plan and providing monitoring and assessment of the patient's condition and response to treatment.
