The shift for this position will be every Saturday from 8:00am-1:00pm.

This will be located at the Lowcountry Senior Center located at 865 Riverland Drive James Island, SC 29412.

Primary Function/General Purpose of Position

The Community Health Support Specialist provides operational and/or administrative support to community health teams in a variety of ways. This role assists with coordination of community health services, which may include assisting with program coordination, data entry/certification, documentation, and/or communication with internal and external stakeholders.

Essential Job Functions

  • Assists with community health program coordination and special projects, as directed by leadership.

  • Provides information and basic guidance to clients about community health program(s), services, and eligibility.

  • Performs support tasks for Community Health teams and leaders, which may include data entry, maintenance, and records management.

  • Supports data collection efforts related to grants and regulatory requirements.

  • Provides exceptional customer service which may include greeting community members, answering and making phone calls, and providing basic services information.

  • Assists with setup of events, meetings, outreach and educational sessions as needed.

  • May support scheduling of client appointments with community services, including coordination of any necessary logistics (such as transportation, interpreters, etc.).

1 year of experience in an administrative support or customer service setting (preferred)
