This is a remote position with some in office work at Palmetto Commerce Parkway, Ladson, SC.

HOURS: 8:00 - 5:00pm, Monday - Friday

Primary Function/General Purpose of Position

The Remote Patient Monitoring (RPM) Specialist supports the day-to-day operations of the Remote Patient Management program. The RPM specialist will assist in monitoring and managing patients participating in the program by reviewing and ensuring submission of vital sign readings and survey responses, addressing client needs promptly, and maintaining accurate health records. Responsibilities include conducting daily patient assessments and delivering regular and written verbal reports to other clinical teammates. The RPM Specialist will work alongside a Registered Nurse (RN) and Advanced Practice Provider (APP) and follow protocols for triage and escalation.

Remote patient monitoring collects patient health data using technology. The data collected includes vital signs such as heart rate, blood pressure, oxygen level and weight. Vital signs are transmitted by the patient and reviewed by a provider. If intervention is needed, patients are contacted. Participants in the program are those with non-acute, chronic conditions.

Essential Job Functions

  • Provides remote patient monitoring including based upon assignment and service expected by client.

  • Utilizes algorithms to determine actions, communication, and escalation and responds accordingly.

  • Communicates directly with patient and or caregiver, clinical staff as well as RPM technology and Biomed resources.

  • Accurately documents in and retrieves information from RPM platform as well as the applicable electronic health record.

  • Ensures compliance to applicable policies and regulatory processes.

  • Assists with data collection and reporting as assigned.

  • Maintains professional collaborative relationships with members of the care team.

  • Participates in process improvement, professional development, peer development, peer review and shared governance.
