Shift: Monday - Thursday 8:00am-5:00pm; Friday 8:00am-12:00pm

Job Summary:

The Outpatient Care Manager will provide care management/coordination services and focused disease education to the community at large, any high-risk population, and contracted populations including members of Medicare Advantage Plans, Physician Provider Networks, and Employer Plan. The Outpatient Care Manager will promote coordinated, high quality, and cost-effective outcomes for assigned caseloads via care coordination and on-going clinical and psychosocial assessments that identifies needs and monitors client progress. Assessments, health education and chronic disease management may be completed telephonically, by direct contact within Clinically Integrated Network partner’s offices, Ryan White Wellness Clinic, Access Health, Transitions Clinic, and/or within the home setting of patients. The Care Manager will develop individualized plans of care to facilitate self-monitoring and self-management skills of health conditions and improve function and quality of life for members while lowering cost.

The Outpatient Care Manager will be an advocate for appropriate resources available in the community and across the continuum of care to best meet the needs of the client. This role will interact with all levels of personnel, medical staff, patients, community resources, providers and families.

The Outpatient Care Manager will work collaboratively with the care team to facilitate efficient and appropriate management of all cases. He/she will balance the department vision with a hands-on approach and will be comfortable driving results independently, as well as with and through others.

Minimum Qualifications:

Education: Bachelor of Science in Nursing, Social Work, Public Health, or related field required. Master’s degree preferred.

Experience: At least two years of experience in a clinical setting. Care management experience preferred. Ability to speak Spanish is highly preferred. Public Health, Medical Social Work or Public Health experience strongly preferred.

Licensure/Certification:

· Appropriate licensure, certification or registration in the profession field of the applicant if applicable. If RN, must be currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC.

· Valid drivers license and eligible for Roper St Francis Healthcare insurance coverage (required).

Knowledge/Skills: Adheres to the CMSA Standards of Practice for Case Management. Knowledge of care management concept along the continuum. Knowledge of Medicare benefits. Ability to successfully articulate the process of attaining goals and outcomes of care management. Ability to apply clinical knowledge in a care management role. Understand and complies with regulations and recommendations of outside regulatory bodies with regard to providing case management services. Excellent verbal and written communications skills. Ability to engage and collaboration with the patients and significant others in the care management process. Knowledge of HIPAA and the ability to apply during the care management process. Demonstrates ability to prioritize activities on a daily basis and adapt effectively to changing priorities. Ability to care and mange diverse populations without applying one’s own personal values. Ability to work with minimal supervision within nursing scope of practice. Ability to think critically and analytically and work with minimal supervision. Clinical knowledge and ability to educate clients of all ages about the following core disease management issues including but not limited to Diabetes, CAD, Asthma, COPD, and renal disease required.
