Job Summary:
The Case Management Associate works under the direction of the department leadership team to provide administrative support for case management processes. The associate performs tasks identified by the case management team for discharge planning, utilization management and care coordination support. This position does not provide any clinical or professional intervention. This position maintains a high productivity level to maximize the overall productivity of the department. He/she supports the care coordination team to expedite appropriate, cost effective, and safe quality care that optimizes patient flow and promotes an integrated and seamless patient-centered experience.
Minimum Qualifications:
Education: High school diploma with two years related work experience in a medical or administrative setting; or an Associate degree and one year in a position involving related work, preferably in a hospital setting required; Associate Degree preferred
Experience: Understanding of health care/case management services and terminology preferred.

Knowledge/Skills: Must demonstrate a strong track record of building and sustaining stakeholder and core team relationships. Requires excellent interpersonal and communications skills. Computer and data entry knowledge essential. Proactive initiation of tasks related to assigned job duties.

Ability to work in a fast pace team environment. Ability to prioritize and multi-task and act professionally under pressure. Ability to develop and maintain positive relationships with entire healthcare team. Ability to maintain confidentiality of sensitive patient information. The ability to navigate within a service orientation and a high level of humility are requirements for successful assimilation into our highly collaborative, patient-centered culture.
Contacts: Frequent interaction with healthcare team, patients/families/caregivers, post acute care services, vendors, and insurance companies.
Work Demands/Environment:
Frequent sitting, walking, standing, stooping, bending. May require lifting or moving items up to 25 lbs. Frequent use of finger/hand dexterity. Constant talking or hearing. Corrected hearing and vision to normal range. Minimal risk of exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Requires work under stressful conditions, deadlines, and/or irregular hours. Ability to read and comprehend.
