Job Description

Responsible for providing social services and coordinating activities to meet patients' social and emotional needs. Identifies and assesses client cases and anticipates their needs related to illness, hospitalization, finances, education and home or community services. Serves as a liaison between health care facilities, community resources and government agencies.

Additional Job Description

Education: Master's or Doctorate degree in social work from an accredited program. Work Experience: 4-6 years

Licensure: Current SC LISW license
