Job Description Summary

The Ambulatory Clinic RN Lead I reports to the Ambulatory Service Manager. Under general supervision, the Clinic Lead Nurse provides individualized, goal-directed patient care to families and patients at the competent level utilizing the principles and practices of the nursing process; delivers safe and effective care and interacts with other members of the health care team to achieve desired outcomes. The Clinic Lead Nurse works in coordination with the nursing leadership. This position ensures the achievement of optimal evidence-based patient care and department goals through efficient daily operations and compliance with established rules, regulations and guidelines. The Clinic Lead Nurse assists with providing leadership presence and staff support.

Job Description

Job Responsibilities

20% Job Responsibility: Clinical Practice Assessment Planning, Implementation and Evaluation

Utilize the nursing process to facilitate age-appropriate patient care coordination activities based on individual patient needs in a safe environment.

Success Criteria:

  • Assess and interpret findings for vital signs, and other required body measurements.

  • Assess and document findings from positive required screenings completed for the individual patient population.

  • Evaluate and document patients’ responses to plan of care, pain, medication usage, follow-up tests, procedures, and treatments.

  • Coordinate care with interdisciplinary team members.

  • Demonstrate knowledge of hospital policies, procedures, nursing standards, and legal issues in areas of specialty.

  • Administer medicine to patients and adheres to rules, regulations and policies related to medication administration and documentation.

  • Adhere to policy for clinic specific medication issues: high alert, pain reassessment, standing orders, med labeling, given in clinic medications and food/drug interactions education.

  • Follow unit and organizational policies and procedures to complete indicated clinical procedures.

  • Assist in the development of an action plan to increase compliance and implement actions to correct identified deficiencies.

  • Identifies and participates in opportunities to share skills, knowledge, and strategies for patient care and system improvement with colleagues and other healthcare providers.

10% Job Responsibility: Patient Education and Teaching

Provide patient instruction that results in increased understanding and improved outcomes.

Success Criteria:

  • Document teaching in EMR following policy.

  • Participate in updating clinic specific education materials.

  • Provide and document education specific to the reason for visit/patient problem, including given in clinic medications.

  • Provide education and teaching to patients and families for medications and medication changes.

20% Job Responsibility: Coordination of Care

Coordinate continuation of plan of care utilizing all methods of communication between patient and clinic.

Success Criteria:

  • Adhere to unit protocols used for phone triage.

  • Utilize EMR phone templates to document patient calls, plan of care, prescription refills and response to call.

  • Remain within scope of practice and appropriately refer call to provider for response or signature.

  • Maintain efficiency with call response time and request for medication refills and other patient requests.

  • Monitor in basket messaging and communicate all relevant information required to coordinate care.

  • Complete and coordinate all relevant tests and procedures required for individual clinic specialties.

Facilitates departmental and/or organizational educational opportunities.

Success Criteria:

  • Recognize learning needs and obtain guidance to acquire appropriate education and/or training.

  • Proactively contribute recommendations to improve patient care and clinic operations.

  • Encourage, advocate, and assist team with Safety reporting (Sheild), investigations and resolution.

  • Collaborate with manager to support education and team actions plans to meet Accountable Care Organizational goals.

  • Coordinate and manage care by applying analytical reasoning, reflection, rational problem-solving skills, using evidence-based information and clinical judgment.

  • Assist with the development of personal objectives.

  • Evaluate practice through discussion with appropriate staff and self-evaluation.

  • Accept and utilize constructive feedback to improve performance.

  • ·Assists with the coordination of competency-based orientation for licensed and unlicensed staff in the Unit.

  • Responsible for selection and roll-out of annual clinical competencies.

  • Uses formal and informal methods to teach

  • Identifies and participates in opportunities to share skills, knowledge, and strategies for patient care and system improvement with colleagues and other healthcare providers.·

  • Assures that areas of responsibility adhere to relevant safety, regulatory, certification and/or accreditation standards (OSHA, JCAHO, DHEC, etc). MUHA vs. MUSCP clinics

  • Demonstrate effective use of computer-based information for decision support, continuous quality improvement and professional development.

10% Job Responsibility: Identify and understand the roles, scope, and responsibilities of the clinic staff.

Success Criteria:

  • Utilize appropriate channels of communication for problem solving.

  • Contribute to unit development by supporting change and offering suggestions.

  • Demonstrate an awareness of cost effectiveness.

  • Demonstrate a professional commitment to patients and staff by adhering to clinic procedures, MUSC policies, and scope of practice.

  • Verify accurate documentation in EMR to enhance revenue collections.

  • Serves as a resource to various populations for the for the purpose of influencing the delivery of healthcare and the formation of policy.

  • As needed, participates in direct patient care to maintain clinical skills

  • Resource to manager/lead/charge nurse and RN III

  • Verify accurate documentation in EMR to enhance revenue collections.

  • Assist with employee rounding and evaluations.

  • Provide coverage during the Manager’s absence to address department-specific concerns.

  • Collaborate on department staffing schedules and monitor daily staffing levels to adjust accordingly for unity acuity and census.

  • Assist staff to work required scheduled hours and days adhering to time and attendance policies (i.e., unapproved overtime).

Additional Job Description

Minimum Training and Education

Associate Degree of Nursing and a minimum of one year of work experience as a registered nurse are required. Previously Ambulatory experience preferred. Bachelor of Science in Nursing Degree preferred RN staff hired on or after July 1, 2013 with an Associate or Diploma degree in nursing are required to be enrolled in an accredited BSN program within two years and successfully obtain a BSN degree within four years of the RN hire or reclassification date. Refer to policy A141 for more details.


Required Licensure, Certifications, Registrations

Current South Carolina licensure as a registered nurse or compact state required. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider. ACLS if required by clinic.

Physical Requirements

Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Ability to be qualified physically for respirator use, initially and as required. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift, lower, push and pull and/or carry objects weighing 50 lbs (+/-) unassisted, exert up to 50 lbs of force, lift from 36” to overhead 25 lbs. Infrequently work in dusty areas and confined/cramped spaces.
