Job Description Summary
The RN collaborates closely with APPs, physicians, and other members of the multidisciplinary team to triage patient concerns, manage Epic in-basket messages, coordinate diagnostic testing, review results, and support transitions of care. This role requires strong clinical judgment, excellent communication skills, and the ability to anticipate and address patient and team needs in a proactive, organized manner.In addition to direct patient interactions, the RN serves as a key resource for workflow efficiency, patient education, and quality improvement initiatives within the heart failure program. By bridging communication between patients, caregivers, and providers, the RN ensures that care is delivered safely, effectively, and with compassion.
Work Schedule: 8am-430pm, Mon-Friday (40 hours/week)

Job Description
Job Responsibilities for Heart Failure/VAD Clinical Support RN
- Collaborate with Heart Failure APPs, physicians, and the multidisciplinary care team to coordinate safe and timely patient care.
- Monitor and manage Epic in-basket messages, including patient inquiries, test results, and prescription requests, ensuring prompt response and appropriate follow-up.
- Triage patient symptoms and concerns remotely, providing education, guidance, and escalation to APPs or physicians when needed.
- Coordinate and track diagnostic testing, imaging, and lab results; ensure patients and providers receive timely communication of results and care plans.
- Support transitions of care by assisting with discharge planning, follow-up scheduling, and patient/caregiver education.
- Serve as a point of contact for patients and families, offering education, resources, and reinforcement of care instructions.
- Participate in program quality initiatives and workflow improvement projects to enhance efficiency and patient outcomes.
- Maintain accurate documentation in the EMR and uphold compliance with hospital policies, regulatory standards, and evidence-based practice guidelines.
- Provide orientation support and mentorship to new staff or team members as needed.
- Demonstrate flexibility and adaptability in a primarily remote role, maintaining excellent communication with both patients and colleagues.
- Organize and prioritize nursing care plans, considering the needs of the patients and other staff members.
- This role collaborates with the APP and Coordinators to provide the most cost-effective and appropriate care for the advanced heart failure patient populations in person and via telephone triage.
Minimum Education and Requirements: Bachelor of Nursing degree preferred. RN staff hired on or after July 1, 2013 with an Associate or Diploma degree in nursing are required to be enrolled in an accredited BSN program within two years and successfully obtain a BSN degree within four years of the RN hire or reclassification date. Refer to policy A141 for more details. A minimum of one year of work experience as a registered nurse required. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required
