Job Description

As a Nurse Home Visitor, you will provide comprehensive, in-home community health nursing services to first-time pregnant women and their families enrolled in the NFP program. You will play a critical role in promoting maternal health, improving pregnancy outcomes, and fostering healthy child development—starting from pregnancy through the child’s second birthday.

This position requires a high standard of clinical nursing practice, a commitment to the NFP model, and strict adherence to the policies, procedures, and quality standards set forth by both MUSC and the NFP National Service Office (Changent). The Nurse Home Visitor works under the supervision of the NFP Nurse Supervisor and functions as a vital member of a supportive and mission-driven team.

Job Duties:

1) Clinical: Complete all required NFP education. Successfully achieve reliability with DANCE (Dyadic Assessment of Naturalistic Caregiver- Child Experiences). Develop a therapeutic relationship with women and their families in a home visiting environment. Perform home visiting in accordance with the NFP model and guidelines. Assess physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains. Assist women and their families in establishing goals and outcomes. Provide education, support and referral resources in assisting women and their families in attaining their targeted goals. Consult and collaborate with other professionals involved in providing services to women and families. - 35%

2) Provide home visits to women and their families eligible for the NFP Program. Adhere to the NFP model of home visitation. Carry a caseload of at least 25 clients. E - 25%

3) Administrative: Support policies, procedures, guidelines and standards of NFP and MUSC. As requested/required, participate in community coalition in support of NFP. As requested/required, promote public awareness of NFP. Develop and maintain community relationships to support client referrals. - 10%

4) Actively engage in skill building to meet all NFP Nurse Home Visitor competencies and requirements. Provide information to support staff for timely and accurate data input to the NFP database (Flo). Cooperate in review and analysis of the Flo reports for achievements and areas for improvement. Participate in quality improvement efforts. Document appropriately and timely. Perform related duties as assigned or required. E - 10%

5) Communication: Maintain clear, effective, open, honest communication with both internal and external customers and colleagues. Create, maintain, and support a safe environment for open discussion. Maintain confidentiality. Seek and respond appropriately to feedback. - 10%

6) Team: Understand, support and coach others in the NFP vision, mission and model. Represent the NFP vision, mission and model in actions and verbally to both internal and external customers and colleagues. Assist in creating a positive work environment that promotes productivity, mentoring, teamwork and cooperation. Elicit and consider differing viewpoints when analyzing issues. Recognize accomplishments of team members. - 5%

7) Professional Development: Accurately assess own learning needs and develop strategies to meet them. Motivated to utilize computer for long-distance learning. Stay informed of current health care developments to provide safe, quality nurse home visiting services. Establish and record progress toward annual goals. Obtain 20 contact hours per year per agency policy. Other duties as assigned. - 5%

Min Requirements:

ADN required, BSN preferred. Current RN license (in good standing) required. Current CPR required. Bilingual in Spanish/English preferred. Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred. Relevant life experience will also be considered. Must be able to work flexible hours.

Knowledge / Skills / Abilities

Applicant must have good organizational skills and strong written and oral communication skills. Ability to develop and maintain positive, therapeutic relationships with diverse groups. Proficient in computer skills, Microsoft Word, Power Point, and Excel preferred.

Supervision Guidelines

Must take initiative and be able to plan and organize work. Is responsible for reporting to the Supervisor of the NFP program.

Follows program processes and reporting requirements in accordance with Nurse Family Partnership model and guidelines.

Additional comments

Travel as required by program. Must be flexible with ability to work weekends and evenings as needed. Position requires home visitation throughout the community and does require use of a personal vehicle.

Meet with NFP Nurse Supervisor weekly for clinical supervision. Utilize reflective practice in supervisory sessions. Schedule joint home visits with NFP Nurse Supervisor quarterly. Attend and participate in bi-weekly case conferences. Attend and participate in bi-weekly staff meetings.

Additional Job Description

Minimum Requirements: Graduation from an accredited school of nursing and one year related nursing experience. Licensure as a registered nurse by the South Carolina Board of Nursing. Some positions require a valid driver's license. Requires certification in Basic Life Support prior to the completion of general orientation. Some positions require certification in Advanced Cardiac Life Support.
