Job Description Summary

The RN Coordinator for Palliative Care plays a pivotal role in enhancing patient care by coordinating daily clinical operations of the inpatient palliative care service, educating staff, patients, and families, facilitating interdisciplinary communication, and supporting patients, families, and staff through complex care transitions. This position ensures timely access to palliative services, promotes goal-concordant care, and contributes to initiatives aimed at improving quality of life for patients with serious illnesses. This position is focused in MUSC--Charleston

Job Description

Consult Coordination:

  • Manage the palliative care consult pager during weekdays.
  • Triage incoming pager consults and effectively prioritize clinical resources to maximize appropriate level of care, consult responsiveness, and team efficiency.
  • Collaborate with primary teams and unit-based staff to identify and review patients who may benefit from specialty palliative care and facilitate timely consults.

Interdisciplinary Team (IDT) Leadership:

  • Lead structured daily Palliative IDT rounds to ensure efficient and effective team communication and operations.
  • Facilitate multidisciplinary goals-of-care meetings, ensuring inclusion of patients, families, and relevant care team members.
  • Attend external IDT meetings as requested for shared patients

Advance Care Planning:

  • Identify medical decision makers
  • Promote the identification and documentation of Health Care Power of Attorney (HCPOA) and advance directives.
  • Ensure HCPOA documentation is current and accessible in the patient's medical record.
  • Provide HIPAA-compliant video links for remote participants in care discussions.

Transition of Care:

  • Coordinate transitions from inpatient to ambulatory palliative care, including patient and caregiver education on palliative care.
  • Identify patients who would most benefit from expedited ambulatory services following inpatient admission
  • Ensure continuity of care especially for palliative care patients with prolonged admissions to identify need for engagement of palliative care IDT members and re-evaluation of goals of care at inflection points in the hospital course.
  • Support inpatient telehealth as needed to ensure smooth care transitions including connection with ambulatory services

Specialized Support:

  • Serve as a contact, resource and coordinator for palliative care patients who may benefit from General Inpatient (GIP) level hospice care.
  • Provide coverage for Left Ventricular Assist Device (LVAD)
  • Assist with HCPOA requests when social work or chaplaincy services are unavailable.

Additional Job Description

Education:

  • Offer palliative care education to bedside staff and medical teams.
  • Collaborate with unit-based case managers to identify patients appropriate for consultation and act as an educational resource.
  • Assist with implementation of appropriate palliative care clinical protocols

Qualifications:

  • Graduation from an accredited school of nursing; BSN preferred.
  • Current licensure by the South Carolina State Board of Nursing.
  • Minimum of three years of medical practice or clinical management experience; other relevant experience may be considered.

Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.

*This is a full-time, onsite hospital position
