Job Description
The CCM Intern will help coordinate aspects of patient care, under the guidance of the pharmacist, including but not limited to:
- Monitor medication adherence and promote compliance.
- Provide pharmaceutical knowledge and other medication related considerations such as: drug interactions, use of medication, and any medication safety concerns.
- Evaluate any medication assistance options to promote and optimize guideline driven therapy management.
- Assist in providing comprehensive medication reviews and medication reconciliation.
- Assist in medication consultations and disease state education.
- Establish medication adherence and disease therapy management tools.
- Address any social determinants of health and barriers to the achievement of optimal health goals.
- Assist in remote monitoring of patients’ blood pressure, blood glucose, A1C, and other clinical data points to optimize outcomes.
- Participate as an active member of a multidisciplinary healthcare team.
- Utilize and develop strong communication skills to collaborate and disseminate knowledge related to pharmaceutical care to patients and other health care providers.
