Supports Lung Cancer Screening program by directing patients through the screening, enrollment, and appointment process. Schedules appointments, diagnostic studies and follow up. Provides a critical link to re-connecting with patients to ensure compliance with annual screening visits. Develops and drives initiatives to eliminate barriers to the cancer screening program. Works closely with Lung cancer screening providers to maintain screening registry, track adherence rates, and provide program level reporting and quality improvement opportunities. Working in conjunction with the clinical leadership, the coordinator modifies, develops, and distributes program material as part of community outreach and engagement.Entity

Job Description
Responsibilities:
35% Complete new referral eligibility screening and scheduling:
- Function as a central point of contact for patients, caregivers, and providers
- Screen the appropriateness of every referral to LCS program per screening guidelines.
- Provide patient, family and provider education materials on screening guidelines
- Assess digital literacy and provide instructions on virtual visit if needed.
- Set appointment expectations for patient and family to include information on location, parking, providers, length of appointments, and other pertinent information.
35% Assist with follow up and adherence to annual screening:
- Under the guidance of providers, assist with scheduling scans, appointments and diagnostic follow up, promptly relaying scheduling information to patients.
- Manage annual follow up contacts via phone, letters, and other means to improve adherence rates based on registry data
- Communicate with primary care and other care providers as needed
- Request outside records, imaging as needed.
20% Maintain registry database records:
- Manage of lung Rads 1-2 with data submission to EON registry
- Attend nodule board weekly
- Troubleshoot submission errors to EON
5% Provide quality assessment audits and reporting
- Track adherence rates and report to department
- Provide data for ongoing quality improvement
Additional Job Description
Education and Experience:
- Bachelors Degree and 2 years work experience; or High School Diploma (or equivalent)
- 5 years experience required.
